Why is it important for people with diabetes to get regular exercise?
“There are a number of reasons why it’s important,” says Dr. Rosen. “First, it helps with controlling weight. I don’t think that weight loss and/or weight maintenance can happen without regular exercise.” The second “is that exercise has been shown to improve the body’s sensitivity to insulin. Exercise helps with controlling diabetes in general, even for people who are not overweight.”
What kind of exercise do you recommend?
“The easiest thing for most people is walking,” says Dr. Rosen. “We recommend that people start with about five to ten minutes a day, working up to about half an hour.” When considering any exercise regimen, consult your physician, who may administer a stress test to determine the limits of what you can and should do. Regular walking, however, probably doesn’t require a stress test, although it may be in order for anything more vigorous. “Starting out slowly is important,” Dr. Rosen adds. “Nobody should be getting injured.”
What are common obstacles to physical activity for people with diabetes?
“Making the time to do exercise is probably the first one,” he says. “It’s important for people with diabetes to realize that exercise is part of their treatment regimen, and they need to find the time to do it.” Foot injury can be another obstacle. “Some people with diabetes have peripheral neuropathy, which means they may not be able to feel trauma to their feet. This makes infections more likely, which can lead to amputation,” he says. Diabetes is the leading cause of non-traumatic amputations in the United States. “It’s important to inspect your feet and shoes for foreign objects, such as stones. I even know someone who stepped on an insulin syringe! Anything like this delivers trauma to the feet.” Make sure that your shoes fit well and that you are wearing socks.
Are there any risks to exercise?
Dr. Rosen says, “The major immediate risk is injury, along with people thinking, ‘Oh, I want to exercise, so that means running a marathon.’ It certainly does not; it means walking and building up to the point where you can start a running program, if appropriate.” Starting an exercise regime too strenuously can lead not just to injury but to cardiovascular and other problems. “There are risks of developing angina, heart pain or a heart attack in response to overly vigorous exercise,” says Dr. Rosen. “A stress test is necessary for any exercise program that is more vigorous than regular walking. “Make sure you time your exercise appropriately in terms of food intake and insulin,” he continues. “Generally, people should exercise after they eat and not before, when the risk of having a severe blood sugar reaction is much greater. Make sure to check your blood sugar before and after exercise, to monitor if you need to eat a little bit more or drink a sugar or sports drink before exercise. Everybody is different in their response to blood sugar.”
Is there exercise clothing made specifically for people with diabetes?
“Not really. The major issue is really footwear,” says Dr. Rosen. “Wear good athletic socks; there are no special socks for diabetes, per se. Other clothing should be appropriate for the temperature outside.” Hot weather can be especially problematic, because of the risk of dehydration: “Blood sugar can increase dramatically when someone gets dehydrated.”
What about diet?
“That’s something to discuss with your nutritionist or physician,” says Dr. Rosen. “Again, eat before exercise to prevent a low blood sugar reaction.”
Is there anything else—besides diet and exercise—that people with diabetes can do to live a healthier life?
“Appropriate nutrition and exercise are the mainstay of diabetes treatment. Along with that,” says Dr. Rosen, “is taking medications as prescribed. It depends on how well someone’s diabetes is controlled to assess what medications are necessary. People with diabetes need to have a relationship with a physician to help to guard their care.”
